Do something for him that's a surprise. Something he would never see coming. It'll definately make him feel special and that it's his birthday, not just valentine's day.

does he like cologne? how about his fav. magazine? if he has a fav. sports team, you could find a t-shirt, or hat w/it on it. are you a couple? if so, maybe buy a single red rose, if not, maybe ask one of his buddies if he would mind getting a flower from "a girl" or if he would just be embarrased by it.
